What it is

Survodutide (development code BI 456906) is a dual agonist of the glucagon and GLP-1 receptors, developed by Boehringer Ingelheim [8].

Research status

In a 46-week phase 2 obesity trial of 387 adults, average weight loss ranged from 6.2% to 14.9% across doses, compared with 2.8% on placebo; gastrointestinal adverse events affected three-quarters of people on survodutide [8]. In a 48-week phase 2 trial in metabolic dysfunction-associated steatohepatitis (MASH) with liver fibrosis, 43% to 62% of participants on survodutide had an improvement in MASH with no worsening of fibrosis, compared with 14% on placebo [9]. The phase 3 SYNCHRONIZE-1 trial, published in 2026, reported average weight loss of 12.2% to 13.0% at 76 weeks versus 5.4% on placebo, with gastrointestinal symptoms in 81% to 90% of participants on the drug [10].

Regulatory status

Survodutide is not approved in the US, the EU, the UK or Canada [1][3][2][5]. It is not named in Australia’s Poisons Standard [6]. It is not named on WADA’s 2026 Prohibited List, although WADA’s section S0 bans substances with no current approval for human therapeutic use [7].

Quality and testing considerations

Because no approved product exists, any survodutide sold online is an unapproved product. The FDA warns that unapproved GLP-1 drugs have not been reviewed for safety, effectiveness or quality [11], and that drugs sold outside regulatory safeguards “may be contaminated, counterfeit, contain varying amounts of active ingredients, or contain different ingredients altogether” [12]. Identity confirmation by mass spectrometry and a measured purity and content for the batch are the minimum evidence of what a vial holds.
